SR50 - This form is used by a person to provide proof of current insurance to the Indiana Bureau of Motor Vehicles. The form shows the Bureau the begin and end dates of the current policy.

If your license has been suspended because there is a second occurrence, of any of the incidents listed below within three years, there is an additional one year suspension. Suspensions and Reinstatement Requirements

Any driver who:

  • Is involved in a traffic accident and cannot provide proof of financial responsibility
  • Has had three traffic ticket convictions in which points are assessed within a twelve month period; the individual must provide proof of financial responsibility on the third ticket.
  • Is convicted of a misdemeanor or felony involving a motor vehicle
  • Has been previously suspended for not having insurance...

Must show proof of financial responsibility, i.e. insurance for the date of the incident. This Certificate of Compliance must be signed by the insurance agent verifying proof of insurance for the date of the incident. The penalty for non-compliance is a 90 day license suspension for each incident and a graduated reinstatement fee as follows:

  • 1st offense $150.00
  • 2nd offense $225.00
  • 3rd offense $300.00
  • Each additional $300.00

If you had insurance at the time of the violation. You need to call the insurance company or your agent that insured you at the time of the incident. They can provide the Certificate of Compliance for the Bureau. .
